What types of cases do you evaluate?
We evaluate workers’ compensation cases, including SIBTF (Subsequent Injuries Benefits Trust Fund) matters, comprehensive medical evaluations, and specialty evaluations.
How do I submit a referral?
Referrals can be submitted by email. Once received, our team will review the referral and begin the intake and scheduling process promptly.

What records are required?
We request any available medical records related to the injured worker’s claim. If additional records are identified as necessary, our team will coordinate directly with the requesting attorney’s office.
What is the typical turnaround time?
- Scheduling: Appointment dates are typically provided within 48 hours of receiving the referral request.
- Evaluation Reports: Reports are generally completed within 45 days, based on the records available at the time of evaluation. Any supplemental records received after the evaluation will be reviewed and addressed as appropriate.

What should I expect during my evaluation?
During your appointment, the doctor will review the information you provided in your questionnaire and ask follow-up questions to ensure accuracy and completeness. This includes discussing your injury, symptoms, medical history, and any relevant pre-existing conditions. If additional information is needed, you will be advised during the evaluation.
How long will my appointment take?
- Initial evaluations: Approximately 1.5 to 3 hours
- Specialty evaluations: Typically 1 to 3 hours, depending on the specialty and complexity of the case
Treatment-related information will be provided as applicable.
Do I need to bring anything?
Please bring:
- A list of your current medications
- For additional documents/records: Please consult with your attorney if any medical records or information have not already been provided.
Is this appointment confidential?
Yes. All medical evaluations are conducted confidentially and in accordance with applicable privacy laws.
What if I need to reschedule?
If you need to reschedule, please notify our office as soon as possible, preferably at least one week in advance.

Frequently Asked Questions on Administrative & Process
How are appointments scheduled?
Once a referral is received, it is processed by our administrative team and typically scheduled up to 45 days out, depending on provider availability and case requirements.
What happens if records are incomplete?
All medical records received are reviewed upon intake. If additional records are needed, we will notify the requesting attorney and request authorization or assistance in obtaining them. Any records received after the evaluation will be reviewed and addressed during the supplemental reporting process, when applicable.
How is communication handled?
Our team strives to be responsive and communicative. We make every effort to respond to inquiries within 24 hours.
How are reports delivered?
Completed reports are served to the parties’ attorneys and the Office of the Director of Industrial Relations (OD Legal), as required. Reports are typically delivered by mail unless an attorney specifically requests electronic delivery.
What is your cancellation policy?
While we do not require a formal cancellation notice period, we ask that you notify our office as soon as you become aware of a cancellation so we can coordinate appropriately with our providers and staff.
